Release of Information

Release of Information All patients or representatives authorize Breastpump Genie, Breastpump Genie's affiliated service and equipment providers (including but not limited to the Breastfeeding Shop), prescribing physicians, hospitals, clinics, hospital networks, and all other holders of information related to service or equipment provided by Breastpump Genie to, upon request, release information to Breastpump Genie, Breastpump Genie's affiliated service and equipment providers, payer sources, physicians, and any other medical personnel or agencies involved with or relevant to such service or equipment. All patients and representatives further authorize Breastpump Genie and Breastpump Genie's affiliated service and equipment providers to review medical history and payer information for the purpose of providing treatment or equipment.
